How long have you been playing? What strength is your reed? Some mouthpieces are designed to work better with different strengths of reeds.

How old is your reed? Is it in good shape? If you're playing on a reed more than two weeks old, you need to get a new reed. If your reed has any deformities in it, you need to get a new reed.

As for your embouchure, make sure you're playing with a flat chin and that you're pulling the corners of your mouth down almost into a frown-like setting. Make sure you're gripping the mouthpiece firmly in your mouth. A weak embouchure will always help you play flat.

Adjust your embouchure. You also might want to look up in clarinet reference books what notes are sharp or flat naturally on the clarinet so that if you tune to one note (C) a different note will always be out of tune.

I just wrote an answer to your other question about your embouchure. If I'm right about your squeaking problem - that your biting, then that's probably whats wrong here too. Biting will make you sharp as well. Relax that embouchure!
