It is run by thetruth.com, an organization (funded by the Tobacco Lawsuit proceeds) whose mission is to inform the public about the dangers of smoking (often quite aggressively). The commercial features a guitar player accompanying a "singer" who "sings" a song about not being able to sing when they've cut out your larynx. Clearly, to use the term "singing" is questionable, because he can't vary the pitch of his voice through the electronic voicebox, but the commercial is very powerful.
