How can I find this obituary?

I have the name, ss number, date of birth and date of death. I also have the state in which the person died, but it's Louisiana and I don't have the name of the parish. I found this information online back in 2001 but I've since lost it and I'm coming up empty now. Can anyone point me in the right direction? Thanks.

Thank's Genevieve but I don't have an actual death certificate. I wouldn't know the name of the funeral home. I got this info online free, but can't find it now. Anyone else?
