What do i need to do to change my last name from my maiden to my married name, i forgot?

i'm just not sure what paper work i have to turn in, or if i can get it off the computer.

Best Answer - Chosen by Asker: if you're in the US...go to a Social Security office with your marriage licence and tell them you need to change your name because you got married
at the DMV you will need your old license and marriage licence
